Reserve Request Form
The
Instructor Reserve Request Form must be completed
for all materials to be placed on Reserve in the
Library.

Reserve Guidelines
The
completed Instructor Reserve Request Form must
be submitted with the material to be placed on
Reserve.
When
applicable, a Copyright Release Waiver must be
signed. See Copyright Guidelines
for further information.
Materials should display Instructor's Name,
Course Title, Course Name, and copy number
for multiple copies.
Materials should be submitted to the Library before
the beginning of the semester in which the materials
will be used.
A minimum of 2 days will be required for processing
of materials submitted during the semester.
Instructors must pick up all reserve
materials at the end of each semester.
The
Library staff will make every effort to safeguard
reserve materials. All materials submitted will
be affixed with a barcode and security strip.
Please note that
the Evans Library cannot
assume liability for damage or loss of materials
which are instructors' personal property.
Copyright
Guidelines
The following items do not need a
signed Copyright Release Waiver:
Books,
journals, or videotapes purchased by Florida Tech
or Instructor.
Material for which instructor holds the copyright,
such as instructor notes, quizzes/exams, homework
solutions.
Works
for which copyright has expired, or no copyright
protection exists.
Federal Government publications.
Single* photocopies of the following being placed
on reserve for one course for one semester only:
a (short) chapter from a book
an article from a periodical or newspaper
a short story, essay or poem
a chart, graph, diagram, drawing, cartoon
or picture from a book, periodical or newspaper.
*For multiple copies - it depends on the number
of students enrolled, the difficulty of the assignment,
and the time
students have to complete an
assignment.
Notice of copyright will be stamped on all photocopies.
The
Library is not responsible for obtaining copy
permission. Individual instructors must assure
that copyright requirements are met. For

Textbook Reserve Guidelines
Textbooks to be placed on reserve are
taken from the Course Book Information Request
Form that was sent by the department to the Florida
Tech Bookstore.
Each department must send either a print
or electronic copy of this form to the Library.
Textbooks listed on this form will be pulled from
the book stacks, processed, and added to the reserve
collection.
F aculty can request that books be placed on permanent
reserve or regular reserve. Books on permanent
reserve never leave the Library, whereas books
on regular reserve can be checked out for overnight
use.
The Library welcomes donations of course
and related textbooks.
Reserve
Checkout Policies
Instructor reserve materials and textbooks on
permanent reserve may be checked out for Library
Use Only, with a two-item and two-hour limit.
A shorter period of checkout may be imposed during
periods of high demand.
Textbooks on regular reserve can be checked out
for overnight use and must be returned to the
Service Desk no later than one hour after the
Library reopens.
Videotapes may be checked out for one week.

Reserves. The Course Reserves
webpage allows you to search for all materials
on Reserve by Instructor, Course Number or Course
Name.
